A: Could be a few things. Maybe the action needed a faster shutterspeed, and thus caused the blur. I would have to assume you are using a wide angle lens, with that slow shutterspeed.
Could be that you bumped the AF lock and locked it on a

Nikon F5 Review « Mike Westfall Photo
The F5 is brutally fast, smooth, accurate, reliable and big. In it’s time it was a chiseled brick of advanced technology that brought every other 35mm SLR down to its knees, and made other manufacturers scramble to come up with a worthy competitor. I use it today because of its reliablity and ergonomics. It always works. It doesn’t matter if you are in sub-zero temperatures of an ice cave, inside the smoldering oven of a desert, or the humid climate of the rain forest.
The Nikon F5 feels like a dream in my hands. The big rubber grips reassure that I have a firm hold on the camera at all times. This allows me to stabilize my shots better when I am using low shutter speeds with slow film.
Just looking through the oversized viewfinder, and seeing the amazing color, sharpness, and contrast of my subject is a real treat after staring through the newer finders of digital cameras that soak up the marketplace these days. If you use a DX crop sensor camera everyday you will know what I am talking about.
